I think that it is the Captain's responsibility, but the sailor's fault. The Sig's have a saying - never pass a fault. Take that to also mean - accept the responsibility that your rank bestows - no matter what rank you are. On a ship, all members of the crew is responsible - from the Ordinary Seaman, up to the Captain.

It may be the Pte's fault that the gun wasn't loaded, but it should be the CO's responsibility that the battle was lost because of it.
