The captain of a ship is responsible for everything that happens on that ship, regardless of whether or not he was a witness to it or not. If two engine room men fall asleep and the ship blows up, it's the captain's fault.

Maybe Army officers should shoulder similar responsibility, rather than blaming subordinates
