Here's what I did - I phoned the clerk who worked on my pay, and she was able to reproduce my "year-end pay statement", providing all the necessary details also found on a T4 slip.
As I mentioned previously, it's always a pleasure to find a hard-working clerk who refuses to give up when "the system" lets us down (i.e. it would have taken forever to obtain a duplicate T4, however it only took a few minutes to obtain a duplicate pay statement - another example of "the indirect approach").
